Medical Facility Registration

((Medical Facility Permit (MFP) is for LFM REGULATED FACTION ran medical facilities / operations. If you are not a faction, please register a business first and then apply for a Business Medical License.))

This registration should be completed by the facility’s leadership. All information submitted must be accurate and complete. False or misleading information may result in the application being rejected or revoked at a later date.

Before You Apply

  • Ensure you have an active character representing facility leadership.
  • Have details of your education and qualification ready.
  • Be able to describe your facility’s field and services.
  • Know whether your facility has PHMC approval and any faction involvement.
Note: This form concerns the registration of a medical facility only. Script-side licenses and scripted businesses are managed separately where applicable.
