Handicap Parking Permit Request

The Pillbox Hill Medical Center (PHMC) provides residents with handicap parking permits where medically justified. This form allows you to request a permit based on your medical needs. Please ensure you read the information and requirements below before submitting an application.

Legal Basis & Permit Types

Any person who requires the use of a handicapped parking space in the City of Los Santos must apply for and hold a valid Handicap Identification Permit, as stipulated in the Handicap Identification of Vehicles Act of 2021 (YULIA ACT) .

The Pillbox Hill Medical Center issues two types of Handicap Identification Permits:

  • Individual (Permanent) – issued for 5 years if you have a permanent disability.
  • Temporary – issued for a minimum of one month and up to 6 months if you have a temporary disability.

Who Is Eligible?

To be eligible, you must have a mobility disability or similar condition that significantly affects your ability to move around. Legislation defines eligible individuals as those who:

  • Require the use of portable oxygen;
  • Have limited use, or no use, of one or both legs;
  • Are unable to walk 200 feet without stopping;
  • Have neuro-muscular dysfunction that severely limits mobility;
  • Have a Class III or IV cardiac condition (by standards of the American Heart Association);
  • Have a severe limitation in the ability to walk due to arthritic, neurological or orthopedic condition(s);
  • Have restriction(s) because of lung disease to such an extent that forced expiratory volume for one second, when measured by spirometry, is less than one liter, or the arterial oxygen tension is less than sixty mm/hg of room air at rest;
  • Have any other physical or mental impairment that imposes an equal degree of disability, creates unusual hardship in using public transport, and prevents the person from getting around without great difficulty.

You are also eligible if you are legally blind.

Criteria

  • The permit holder must be the person with the qualifying disability.
  • Applications must be based on a genuine, medically recognised condition that significantly affects mobility.
  • PHMC reserves the right to request additional documentation or a medical report before granting a permit.
  • Providing false or misleading information may result in the refusal or revocation of a permit and further action as appropriate.

Minimum Requirements

  • You must be the individual the permit is being requested for.
  • You must provide accurate personal details and contact information.
  • You must provide a brief medical explanation and confirm that a doctor has diagnosed your condition.

Method of Review

  1. Submit your Request – Complete the form with your personal and medical details.
  2. Review of Request – A PHMC staff member will review the application and may request further information.
  3. Outcome – If your request meets the criteria, a handicap parking permit will be issued and you will be notified.
